more information or stop by during office hours between 5p.m. and 7p.m. Mon-Fri.
Documentation Requirements
A copy of your childs birth certificate will need to be turned in to the club before games begin via fax (904) 278-9787
or in person during office hours.

Equipment Required
Shin guards, soccer cleats, ball, and Clay County Soccer Club uniform.
Where to Buy
Recreation League uniforms are purchased
at Sports Mania for $40.00 when practices
begin. Academy uniforms will be handed
out when practices begin in February. All other equipment can be found at any sporting goods store including, but not
limited to, Sports Mania or Dick's Sporting Goods.
Play Up Policy
There will be NO playing up
in the Recreation League. All other leagues must contact their respective directors for more information.
Refund Request
Refunds will only be granted in the following 2 cases:
1) If the child is injured during the season
of play parents may apply for a refund with proper documentation, i.e. Doctors note. Application must be filed before
the end of the season.
2) If the family is relocation out of the state or out of the Jacksonville area parents
may apply for a refund with the proper documentation, i.e. Transfer Orders.

Forms Payment
Coaches/Volunteers - Background Check
The forms to fill out for the background check must be picked up in the
office.
